Immunization records and reporting (vaccinations)
Children require certain vaccinations to attend school or child care and early years centres, or they require a valid exemption. Parents and guardians must keep their child’s record up-to-date with the Health Unit.
Adults are also encouraged to report their vaccinations to the Health Unit.
Options to update your vaccination record:
- Online reporting:
- Immunization Connect Ontario is a web-based service provided by the Ministry of Health and Long-Term Care that allows you to submit immunization information. You can also access your immunization record using a unique Ontario Immunization Identification (OIID) and a Personal Identification Number (PIN). Report or access your immunization information.
- Bring a copy of the vaccination record to any Health Unit location during regular business hours.
- Call the Health Unit at 705.522.9200, ext. 458 (toll-free 1.866.522.9200).
- Fax a copy of the vaccination record to 705.677.9616 (confidential fax).
- Mail a copy of the vaccination record to:

If your child requires an exemption from vaccination for medical, religious, or conscientious reasons, you must submit one of the following forms, which are available online at www.forms.ssb.gov.on.ca (Government of Ontario):
Statement of Medical Exemption
- Have a physician or Nurse Practitioner sign the form and indicate the reason for the exemption.
- Submit the form to the Health Unit
Statement of Conscience or Religious Belief
- You will need to have the exemption form witnessed by a Commissioner for Taking Affidavits.
- In addition to submitting this form, parents and legal guardians must also complete an educational session on immunization at one of the Health Unit’s offices (except the Rainbow Centre location). Please contact the Sudbury & District Health Unit to book an appointment.

Before reporting vaccinations to the Health Unit, please read this information.
All of the information you submit is confidential and stored on a secure provincial immunization database (Ontario). It is important for everyone to keep track of their personal immunization record.
The Health Unit maintains and reviews immunization records for everyone including children in all schools, licensed child care and early years centres within Greater Sudbury and the districts of Sudbury and Manitoulin.
The information on children is collected and disclosed by the Sudbury & District Health Unit within the authority of the Immunization of School Pupils Act, the Health Promotion and Protection Act, and the Child Care Early Years Act, to:
- Review a child’s immunization record during an outbreak of a disease that would be preventable through immunization to determine if they are protected.
- Annually assess a child’s immunization status to ensure they meet the requirements to attend school or licensed child care or early years centres.
- Share a child’s immunization information with their school principal once suspension orders have been mailed to parents.
- Share a child’s immunization information with their licensed child care or early years centres operator when:
- The child’s immunization information is incomplete and the parent or guardian has not provided the required information to the Health Unit.
- There is an outbreak or risk of an outbreak of a communicable disease and unimmunized or under-immunized children may be required to be excluded from attendance.
- A copy of a child’s immunization record needs to be sent to another public health unit if a child moves to a school in another public health unit’s service area.
